1. Birth Certificate

• Student’s name on birth certificate must match name on the Scholarship Request Form.

2. Proof of Address

• Current (within 30 days) utility bill showing both the service address (top of bill) and billing address

• Make sure ALL pages are attached. Even if a blank page is shown, all pages must be submitted. 
• If a utility bill is unavailable, click HERE for acceptable forms of Proof of Address

3. Limited Power of Attorney

Complete a copy of the required Limited Power of Attorney here:

• • Limited Power of Attorney specifically authorizes Agaepe Christian Academy (ACA) to endorse and deposit checks issued by the State of Ohio Educational Choice Scholarship Program (EdChoice).
  • All funds will be applied directly to your account.

Print, sign, scan and upload this form. Wet signature is required; typed signature is not accepted.

Once we process your documents from the submission in step one, ACA will contact you with instructions to complete income verification.

EdChoice Expansion Applicants must complete and submit income verification through the Department of Education website. An OHID account is required to complete this step. Instructions to create an OHID account will be included in the income verification communication. Income verifications do not come to Agape Christian Academy, but are used by the Department of Education to determine your award.

Awards may not be determined for several weeks. Once all steps are completed, you wait for state notification via mail.

The State of Ohio will mail you a Scholarship Acceptance Letter once your scholarship is approved. When your EdChoice Scholarship is awarded, you will be notified by mail from the state. To finalize your EdChoice process, please complete the letter you receive in the mail and return it to ACA using this form:
