I/we consent to the student named above being transported by the Agape Bus between the Burton Campus (14220 Claridon Troy Road, Burton) and the Troy Campus (17791 Claridon Troy Road, Troy Township) in accordance with the schedule outlined above.

We agree to release and discharge Agape Christian Academy, its officers, agents, and employees exercising reasonable care within the scope of employment, from liability growing out of personal injuries and property damage resulting or occurring during the transport to and from Burton and/or Troy.

If a dispute over this agreement or any claim for damage arises, the parent/guardian agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If there cannot be agreement on such a process, the dispute will be submitted to a three-member arbitration panel of the American Arbitration Association for final resolution.
